Under the supervision of the Manager, PI and Optimization and the Lead Nurse Navigator provides services primarily to orthopaedic and spine patients scheduled for surgery.
Responsibilities include assisting patients with enrollment in My Chart and with use of Care Companion, tracking engagement with Care Companion data collection, assisting patients with completion of surveys, reporting clinical issues to navigators, trainings and meetings.

Responsibilities:

Evaluates engagement with Care Companion and intervenes with patients who are not using system consistently.

Assists patients with use of My Chart and Care Companion 2.
1Contacts patient and walks through steps to sign up for My Chart and accessing Care Companion 2.
2Maintains data on those patients who will not use My Chart 2.
3Monitors use of My Chart and Care Companion 2.
4Follows up with patients who are not using system 3.

Assist patients with completion of surveys 3.
1Monitor completion of surveys 3.
2Contact patients who have not completed surveys 3.
3Assist patients with completion of surveys if necessary 3.
4Enter survey data into EPIC 4.

Act as liaison between patient and navigator 4.
1Inform Navigator of any patient issues or questions 4.
2 Become familiar with what would constitute and urgent situation for patients 5.

Maintains accurate patient records 5.
1Ensures that all necessary information is documented in electronic medical records 5.
2Documents all patient interactions electronic medical records with accurate notes 5.
3Works collaboratively with the navigators 5.
4Prioritizes daily work with the help of tracking system 6.

Provides additional support to navigators 6.
1 Scan and upload documents into EPIC 6.
2 Assist with faxing information 6.
3 Assist with obtaining information from physician offices
Qualifications:
EDUCATION EDUCATION (number of years and type required to perform the position duties):
High School Diploma or GED required.
Associates Degree in health and/or human services or related field preferred.
EXPERIENCE EXPERIENCE (number of years and type required to meet an acceptable level of performance):
At least 1 year experience in health care or in human services with a focus on patient relations preferred.
LICENSURE NA SPECIAL SKILLS Ability to work independently with support and supervision from Manager and Lead Navigator and to also collaboratively and communicate effectively with others, including patients and families, office staff, and navigators.
Candidate must be culturally competent and be able to thrive in a moderately paced, health care environment where the Orthopaedic/Spine Patient Resource works as a key, valued member of the multidisciplinary team.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
